Referrals and Collaborations:

At A Village Wellness Center, we embrace the collaborative journey toward individual and community wellness. We welcome referrals from licensed professionals and are keen on fostering partnerships to enhance the treatment and care we provide.

Our guiding principle, “know me to serve me,” reflects our commitment to sufficiently understanding each individual’s unique needs and circumstances. In line with this, we may reach out to you, the referring clinician, for additional information. This collaborative approach ensures we can tailor our services effectively and offer the most beneficial treatment.

Referral Information:

For the Psychiatric Rehabilitation Program (PRP): We require referrals for our PRP and depend on your insights to understand the specific needs of those you serve. Please feel free to fill out our referral form and submit it to [email protected]. We also welcome you to email [email protected] to schedule an introduction with our PRP Rehabilitation Counselor or for inquiries about our PRP. 

For the Intensive Outpatient Program (IOP) and Outpatient Program (OP): While referrals are not required, we welcome them. Collaborating with other professionals allows us to provide a more integrated treatment experience. If you have a patient who you believe could benefit from our IOP or OP services, please feel free to reach out to us at [email protected].

Guidance for Self-Referral:

Self-referrals are welcomed for IOP and OP. You can refer yourself by reaching out to us via phone or email ([email protected]) for a consultation. 

While individuals cannot self-refer to our PRP, we are here to assist. If you or someone you know is seeking support, we can help you connect with a therapist in our network. They can evaluate your needs and, if appropriate, refer you to our program. We are committed to making the referral process smooth and ensuring that everyone gets the appropriate level of care.
