Designed for both adults and youth aged 10 and above, our program aims to equip individuals with the skills and resources needed to manage their mental health conditions while navigating everyday life effectively.

Community reintegration is important in rehabilitation. AVWC helps you develop community (aka "village") responsibilities and relationships.
Who We Serve
Adults (18+)
Our program accepts adults with mental health referrals from a licensed practitioner.
6 sessions per month
Hybrid model: in-person and virtual
Minors (Ages 10-17)
Our program accepts minors with mental health referrals. Parental agreement and engagement is required.
3 sessions per month
Hybrid model: in-person and virtual
Free for eligible Maryland Medicaid recipients. Our treatment plan considers the whole person and their past as we work with you towards a more fulfilling future.
How Our PRP Works
Referral from a Licensed Mental Health Practitioner
Maryland Medicaid requires a formal referral from a licensed mental health professional. If you don't have a therapist, we can help connect you with one. If you have an eligible diagnosis, your therapist can refer you to our program.
Assessment & Goal Setting
Once referred to our program, we begin with a comprehensive assessment. We work to understand your unique needs, strengths, and goals to create a personalized rehabilitation plan.
Care Champion Matching
Based on your assessment, we match you with a dedicated Care Champion who will work closely with you. Adults receive 6 sessions per month; minors receive 3 sessions per month.
Skill Building & Progress
Through individual and group sessions, you develop practical skills for daily living and wellness. We celebrate your progress and adjust your plan to support continued growth.
Am I Eligible?
PRP services are available to Maryland residents who meet certain criteria. Our intake team can help determine your eligibility.
